On Monday, September 22, 2025, a sad incident happened in Columbus, Ohio. 3-year-old Cartier Smith accidentally shot himself after finding a loaded gun in his mother’s apartment.
His mother, Carley Dials, 29, is now facing charges of involuntary manslaughter, as per Law&Crime.

According to the local Police department, officers responded to a 911 call from Dials around 9 a.m. on that fateful day. When they arrived, they found young Cartier with a gunshot wound to the head.
He was rushed to Nationwide Children’s Hospital but sadly, he was pronounced dead less than an hour later.
According to police reports, the weapon had been brought into the home by 31-year-old Sean Perrin, who Dials had brought to her home.
Police also said that Perrin had prior gun-related convictions before this incident.
When she was questioned, Dials told investigators she found the gun in her home around 3 a.m.
However, instead of calling authorities, she hid it between her mattress and the wall before going to sleep.
Hours later, Cartier reportedly climbed onto the bed where his mother and Perrin were sleeping, discovered the gun, and pulled the trigger.
As police documents outline, “The child gained access to the firearm and unintentionally discharged it, causing a fatal injury.”
Immediately after the shooting, Dials called 911, but Perrin fled the apartment before police arrived. Authorities later arrested him, charging him with having a weapon under disability and violating parole connected to a 2019 gun conviction. He remains in custody at James A. Karnes Correction Center on a $750,000 bond.
Meanwhile, Dials was arrested the same day and booked into the Franklin County Jail. She was charged with involuntary manslaughter and released on a $50,000 bond. Both Dials and Perrin are scheduled to appear in court on October 2, 2025, for preliminary hearings.
Authorities also confirmed that another child in the home has since been placed under the care of Franklin County Children’s Services.
Even though authorities believe this was an accident, they have still pointed out the importance of responsibility.
“When firearms are left unsecured, children are the ones who pay the price,” said one investigator.
